  • You are solely responsible for how and to whom you submit the completed form (e.g. to a pathology provider or genetic counsellor).

We are not involved in the screening process, data submission, or follow-up services provided by third-party labs. If you have a high chance result, you consent to the laboratory contacting our service to provide follow-up services of genetic counselling and medical assessment as required.
